STEP ONE:

Please REVIEW the student handbook below and complete the packet of forms and email along with Immunization records, and copy of driver's license, and copy of social security card OR birth certificate to: stephanie.taylor@hccnetwork.org. We require Hep B, TB, and Flu shot for immunizations.

STEP TWO: 

To prepare for your rotation, we ask that you watch these three brief videos on Federally Qualified Health  Centers and InterProfessional Education and submit a short reflection to this Google form:  PRE-CLINICAL REFLECTION
